REGISTRATION OF BIRTH OUTSIDE  SRI LANKA

    ·         Application for registration of birth should be made within 3 months of the    birth.

 

Parents are requested to forward two sets of original forms and two sets of photocopies with original certificates of the following documents for each child.

 

a.       Prescribed form “B4”(if application is submitted within 3months of the child’s birth)

 

b.      Prescribed form “B6” (if application is submitted after 3 months of the child’s birth)

 

c.        Prescribed form “Citizenship –1” Register of birth outside Sri Lanka.

 

d.      Declaration of Citizenship of the father.

 

e.        Prescribed form “C” signed by both parents.

 

f.        Prescribed form “O” signed by the parents on whose passport the child’s name to be endorsed.

 

g.      Forms “B4” /”B6” and “Citizenship –1” to be completed and signed by one and the same parent. Two signatures are required on the back side of Form  “B4”/“B6”, one over the “informant” and the other is across the cage – no stamp is needed. Do not sign the declaration appearing soon after clause 10 of B6 this has to be completed by the High Commission Official only.

 

h.       Original Birth Certificate of the Child (if in UK the large certificate) in which the names of both parents appear. (Please note: Birth Certificate has to be issued by the competent Authority in the country where the child was born)- Three copies.

   i.    Original Marriage Certificate 2 photocopies.

    j.    Original Birth Certificate of parents with 2 photocopies.

    k.    Valid passports of both parents together with 2 photocopies each, of parents photopages, renewal pages current visa pages and pages 2 & 3. It is essential to forward the Sri Lanka passport possessed by the father (with visas) at the time of the child’s birth.

     l.      3 copies of recent photographs coloured (35 x 45 mm) one of which should be certified on the reverse by a GP or a Solicitor (with the seal)

 

    m.    If the child’s birth was not registered within one year of the birth, a letter signed by both parents, giving      reasons as to why it was not registered within the first year of birth.

 

    n.    A letter signed by both parents giving their consent for the inclusion of name of the child, either in the mother’s or in the father’s passport.

 

    o.    Documents titled “Affidavit relating to paternity and declaration of Citizenship”, if parents are not legally married at time of child’s birth. Any other document that may be required.

Fees

For registration of birth including registration of birth     

    outside Sri Lanka within one year and Issue of Birth Certificate    £ 22.00

For delayed registration of birth outside Sri Lanka with a

     surcharge per year                                                                     £  4.50

For sending Birth Certificate (postage)                                            £  2.00

To endorse the child’s name in the passport                                    £  2.50

 

        PAYMENT BY CASH OR POSTAL ORDERS ONLY

For returning parent’s passports and other relevant documents, please provide

a self addressed prepaid special delivery cover to the correct weight.

 

To endorse a child’s name into a parent’s passport requires a period of 10 days. If the parents have indefinite visa (permanent residence) in the UK such parents are required to submit a letter from the Home Office (Liverpool) to confirm that the parents are not naturalised as British Citizens. Specimen form is available at the counter. If the father is naturalised at the time of the child’s birth, the registration is not possible.

 

Birth Certificates are issued by this High Commission. Once the birth is registered the certificate will be sent by post. Certificate of registration is issued by the Ministry of Defence (Citizenship Division) in Colombo. The parents will be notified once it is received.

 

Submission of forged documents, i.e. Birth/ Marriage Certificates or the National Identity Card and making false declaration relating to current/previous passports or  Citizenship status is an offence and no refund is possible in such instances.
