ISSUE OF NEW PASSPORTS IN LIEU OF

PASSPORTS  - REPORTED LOST OR DAMAGED

 

1.   Application forms for new passports in lieu of lost passports / damaged passports can be obtained personally form the Consular Division or by sending a self addressed stamped envelope with the request or from website: www.slhclondon.org

 

Application Procedure

Please read instructions before completing the applications.

            Important

      (1). Form “K”

  1. All cages from 1 –20 are to be filled. In the second page after the declaration of the applicant, the certificate (attestation) must be done by a Justice of Peace (JP), Attorney at Law / Solicitor or a Medical Practitioner (GP). Attester’s official frank / stamp must be placed below his / her signatures.

 

  1. Four (4) coloured photographs 3.5cm x 4.5cm are needed. Two (02) photographs to be attached to the form “K”. One of them should be certified overleaf by the same JP, AAL/ Solicitor or GP stating “I certify that the photograph appearing overleaf is of Mr /Mrs”. Attester’s signature, date and seal should be placed below the certification.

 

 (ii). Photo signature Card

a.       The form “IM35b” should be attested by the same JP / AAL/ Solicitor or GP, who has signed the form “K”. The other two photographs should be pasted within the borders of two cages on the “IM35b” photo signature card. Do not staple or pin. The Attester’s signature and seal should be placed over the “Verification Photograph”.

b.      The signature of the applicant should be placed well within the two cages. The Attester should sign the declaration and place his/ her seal and date.

 

(iii). Form “D”

 

(iv).  Statutory Declaration, attested by a JP / AAL/ Solicitor.

 

  1. REQUIREMENTS

In addition to the above forms and documents Originals with two (02) full photocopies of the following should be submitted.

a).  Documents relating to the present visa status in UK.

b). Original Birth Certificate (Translations are not considered as original Birth

     Certificates)

c). Written confirmation from the Police to the effect that loss of passport has

      been reported. If lost by Home Office or by post a letter from them.

d). Professionals should produce appropriate professional certificates and a letter from the employer to include such professions on the passport.

e). Original Marriage Certificate with 2 photocopies if the applicant has change the name after marriage.

e). If the applicant or applicant’s father was born outside Sri Lanka, his Citizenship Registration Certificate issued by the Ministry of Defence/ Interior (Citizenship Division) in Colombo. 

  1. FEE £211/- 

Payment of fees could be made in cash at the counter or by postal orders /bank drafts written in favour of the “Sri Lanka High Commission” if sent by post. Please note that personal cheques will not be accepted.

 

  1. If a separate passport is needed for a child whose name was endorsed in the mother’s /father’s lost passport.

 a). A letter signed by both parents giving consent to issue a separate passport to the        child. If parents are divorced or separated original copy of the court decision  with letter of custodianship (two photocopies are required).

b). In case of a guardian applying for a passport for a child, the guardian must produce the legal document to prove the guardianship.

c).  Original Sri Lanka birth certificate with two photocopies.

d).  If the child was born outside Sri Lanka the Citizenship Certificate with 2

      photocopies.

e).  A special declaration signed by mother / father if the child travelled into UK in their passports (Specimen form is available at the counter).

 

  1. IMPORTANT

i). Making an application for an All Countries Passport in lieu of lost or damaged passport does not necessarily mean that you would be issued with a passport. The decision whether to issue an All Countries Passport will be taken by the Controller of Immigration & Emigration in Colombo after perusing the application.

            ii)  If the applicant has entered this country without a prior visa or a passport, an ordinary passport will not be issued. However an NMR (temporary passport)  valid to return to Sri Lanka only, with validity of 6 months can be issued if the applicant so wishes.

            iii) Passports are issued by the Controller of Immigration & Emigration in Colombo. It takes about eight weeks (8) to issue a passport, you will be notified by a letter when the passport is received.

            iv) Every applicant including children have to be present for an interview with                    relevant documents at the Consular Division on any working day between 9.30 am and 12.20 pm.

            v)   Any applicant who is on an indefinite visa for over one year, is required to submit a letter from the Immigration & Nationality Department in UK, to confirm that he / she has not acquired or ha not applied for UK Citizenship.

            vi) All original certificates will be returned to the applicant after perusal.

            vii) Any enquiries relating to the issue of passports should be made between 9.30 am and 1.00pm.

            viii) The High Commission is not responsible for any documents lost in the post.

 

PLEASE NOTE THAT SUBMISSION OF FORGED DOCUMENTS AND MAKING FALSE DECLARATIONS RELATING TO THE CITIZENSHIP STATUS OR REGARDING THE LOST/ DAMAGED PASSPORT IS AN OFFENCE. REFUND OF FEES IS NOT POSSIBLE IN SUCH INSTANCES.
