ISSUE OF NEW PASSPORTS IN LIEU OF

OLD PASSPORT (10 YEARS COMPLETED)

 

Validity of a passport cannot be extended beyond 10 years from the date of its issue. In such cases or if there are no pages for visa endorsements a new passport should be obtained. Applications for new passports should be made well in advance of the expiry of the current passport.

 Application forms for new passports can be obtained personally from the Consular Division or by sending a self addressed stamped envelope with the request or from website: www.slhclondon.org

 

  1. Application Procedure

      Please read these instructions before completing the forms.

 

Important

(i)                 Form “K”

 

a.       All cages from 1 –20 are to be filled. In the second page after the declaration of the applicant, the certificate (attestation) must be done by a Justice of Peace, Attorney –at-Law / Solicitor or General Practitioner (GP). Attester’s Official Frank/ Stamp must be placed below his signature.

b.       Four (4) coloured photographs 3.5cm x 4.5 cm are to be enclosed. Two (02) photographs are to be attached to the form “K”. One of them should be certified overleaf by the same JP/ Attorney-at Law/ Solicitor or GP stating that “I certify that he photograph appearing overleaf is of Mr. / Mrs. …” His signature, date and official Franks / Stamp should be placed below the certification.

     

(ii)               Photo Signature Card

a.       The form “IM35b” should be attested by the same JP, Attorney –at –Law, Solicitor or GP, who has signed the form “K”. The other two photographs should be pasted within the borders of two cages on “IM35b” photo signature card. Do not staple or pin. The Attester’s signature and seal should be placed on the “Verification Photograph”.

b.       The signature of the applicant should be placed well within the two cages. The Attester’ should sign the declaration and place his seal and date.

 

  1. Requirements to be met by applicants

            a.   Duly filled form K, Form D3 and Photo signature card (as above), should  be submitted.

            b.   Any applicant who is on an indefinite visa for over one year, is required to submit a letter from the Immigration & Nationality Department, Nationality Division, 3rd Floor, India Building, Water Street, Liverpool, L2 0QN, to confirm that he /she has not acquired British Citizenship or has not applied for such Citizenship.

            c.   If the applicant is a Dual Citizen, the original Dual Citizenship Certificate with two photocopies have to be submitted.

            d.   Original Birth Certificate with 2 photocopies.

e.       If there is a change in the profession or new profession to be included in the passport, original professional certificate with 2 photocopies and a letter from the employer should be submitted.

f.   Marriage Certificate with 2 photocopies if the applicant has changed the name after marriage. 

            g.    Documents relating to the present visa status of the applicant if it is not stamped on the passport.

            h.   All original certificates will be returned to the applicant after perusal. 

  1. If a separate passport is needed for a child whose name is already in the father’s or mother’s passport, the under mentioned documents should be submitted in addition to Form “K”, photo signature card & form “D3”.

 

a.       A letter signed by both parents giving consent to issue a separate passport to the child. If parents are divorced or separated an original copy of the court decision / guardianship with two photocopies is required.

b.       Current passports of both father & mother should be produced with two photocopies each of the pages that have the child’s name endorsed & have the child’s photo affixed.

c.       In case of a guardian applying for a passport for a child, the guardian must produce the legal document to prove the guardianship.

d.       Original Sri Lanka birth certificate and if the child was born outside Sri Lanka the citizenship certificate with 2 photocopies also should be enclosed.

  

  1. Other Relevant Information
 
    1. Regular passports are issued only by the Controller of Immigration and Emigration in Colombo.  Usually it takes six (6) to eight (8) weeks to issue a passport after submitting an application.
    2. Your current passport should be submitted along with the application. It will be retained at the High Commission and returned to the applicant after cancellation with the new passport.
    3. If the new passport is to be returned to you by post, you should provide a self addressed prepaid special delivery envelope to the correct weight.
    4. Once the new passport is received from the Controller of Immigration & Emigration in Colombo, applicant will be notified by a letter.
    5. The High Commission is not responsible for any documents lost in the post.
    6. Any enquiries relating to the issue of passports should be made between 9.30am and 1.00pm.

     

  1. Fee for a new passport is £ 85/-

Payment of fees could be made in cash at the counter or by postal orders/ bankers draft written in favour of the “Sri Lanka High Commission” if sent by post. Please note that personal cheques will not be accepted.

 

     PLEASE NOTE THAT SUBMISSION OF FORGED DOCUMENTS SUCH AS THE BIRTH

     CERTIFICATE OR THE NATIONAL IDENTITY CARD AND MAKING FALSE

     DECLARATIONS RELATING TO THE CITIZENSHIP STATUS IS AN OFFENCE AND NO

     REFUND OF FEES IS POSSIBLE IN SUCH INSTANCES.
